TRANSITIONAL HOUSING: T&K Transitional Housing Assistance Program is operated with one goal in mind and that is to help individuals obtain permanent housing as quickly as possible. In this housing community, this will allow our clients a comfortable and safe environment, the ability to add structure to their daily life, provide educational guidance about the fundamental life responsibility so that they become productive citizen.
TRANSPORTATION: T&K Transitional Services provide transportation assistance to clients. The transportation assistance program is available to those individual when public transport is not available and may not be able to meet clients’ timely needs. The transportation assistance program provides transportation for job search, job training activities, home hunting, trips to grocery store, appointments, employment and other needed activities that are required by the client. Transportation assistance is available over the weekend.
LIFE SKILLS: Helping those experiencing homelessness acquire life skills that can help move clients from homelessness to resettlement into the community. There are broad areas, the clients will be introduced to: (1) core or basic skills (e.g. numeracy, literacy and information technology), (2) independent living skills (e.g. managing a household, budgeting, appointment keeping and contacting services, dealing with bills and correspondence (written or verbally), and (3) social skills (e.g. interpersonal skills, avoiding or dealing with neighbor disputes, developing self-confidence and social networks).
HEALTH AND WELFARE: Health and Welfare assistance is a program that will connect people that are homeless to Government benefits. T& K Transitional Services will partner with the agency to educate clients on Medicaid eligibility requirement and how Medicaid work, Medicaid Benefits and Service Delivery. We will work hand in hand with our clients and the agency to ensure that clients receive the benefit that he/she qualifies. Medicaid is an important avenue for clients who experience homelessness to secure basic health care services.
RESIDENTIAL SERVICES: Supporting our clients by assisting them in maintain and sustain their residence when they have transition the transitional program. The major goal is to extend supportive services that will assist in the maintenance and sustaining to the clients that have met the exit criteria to ensure that the client maintain their self independent in the community.
SPIRITUAL GUIDANCE: Spiritual Guidance is an informative format where class will be conducted to our clients to give them hope and a sense of belonging. This service will provide a structured spiritual program for encouragement, support, and commitment. Fruits of faith are benefits of religious belief and practice for people. Religious belief and practice have given and will continue to give clients significant psychological, social and emotional benefits. These include a sense of belonging and fellowship within a community of co-believers, where a positive identity is shared. Spiritual Guidance brings order, meaning and ethical direction to people’s lives, often centered on notions of charity and acts of kindness to others. This Faith and belief class will bring consolation and comfort to alleviate the sadness of homelessness and the separation from love ones.
Spiritual Guidance for homeless people is both for those with faith and those who are not conventionally religious but see themselves as spiritually curious, religious belief, and practice will help them come to terms with a past that is often characterized by profound emotional and material loss, enhance and give structure to the present where time hangs heavy for many, and create a purposeful future built on hope, fellowship, and sense of purpose.
TRANSITIONAL EXIT CRITERIA
Clients that complete the majority of these tasks will be ready for transitioning back to the community. Clients must accomplish other core criteria to transition: (1) Client must maintain employment, (2) Client must maintain a stable income/or public benefits, (3) Show that they are capable of performing daily tasks and (4) Find a Permanent home.
